Whole Person Care

Whole Person Care (WPC) is a pilot program within Medi-Cal 2020 California’s Section 1115 Medicaid Waiver. WPC is designed to improve the health of high-risk, high-utilizing patients through the coordinated delivery of physical health, behavioral health, housing support, food stability, and other critical community services.

Goal

Our goal is to enroll eligible persons into the program, provide screening and coordination of services plus new services not funded by Medi-Cal to assist the enrollees in the journey of wellness.

Population served

Whole Person Care Pilot’s target population includes high utilizers of health and behavioral health services. With a focus on homeless, persons on General Assistance, transition aged-youth, and older adults (aged 55+ with certain conditions). The number of WPC enrollees is expected to grow and change, starting with 750 served at the end of 2017 to 5,250 by the end of 2020, reaching over 10,000 unique individuals (the top 5% of HUMS) by the end of the pilot.
