Elena Guzman has been with Community Health Partnership since 2007, currently serving as the organization’s Deputy Director where she leads the vision and operational alignment to achieve CHP’s mission.

Ms. Guzman guides CHP initiatives in core competency of Quality Improvement and advocacy, with an emphasis on Patient Centered Medical Home Learning collaboration for CHP’s 10 member clinics and the County of Santa Clara’s Health and Hospital system.  Additionally, Ms. Guzman has facilitated collaborative efforts with multiple agencies including Kaiser Permanente, California Primacy Care Association, UCLA, and Stanford University.

Although originally from southern California, Ms. Guzman has made the Bay Area her home for over two decades.  An avid cyclist who loves the outdoors, Ms. Guzman is also a strong advocate for women’s and children’s health.
