Blanca EzquerroPROGRAM COORDINATOR II

Blanca Ezquerro is a Program Coordinator II for Community Programs. For over 25 years, she has had the opportunity to provide outreach and education to her community on different health programs available for them in Santa Clara County.
She has been working with Community Health Partnership for 12 years.  During all these years at CHP she has had the opportunity to learn and to participate in the development, implementation and education on health programs available in Santa Clara and San Mateo counties. In addition to outreaching into needed communities, she had enrolled CHC’s patients and the community into different health coverage programs through Covered California, MediCal and PCAP.
Currently, she is facilitating the Chronic Disease Self-Management in Spanish and provides support for the Ventanilla de Salud at the Mexican Consulate in San José.
She came to this country as an immigrant, without resources and with younger children lacking health coverage and not knowing how to access services therefore,  she understands how challenging and worrisome it is to access health services. She believes in the importance that every person should have access to affordable care in their communities and she is grateful, for all these years that she has the opportunity to share that there is a health coverage program available to them.
